HOW YOU BECOME A CLIENT COMPANY

Fill out the Client Application and Client Information forms (PDF), print, sign and mail or fax to the program manager. These forms are available online, but if you prefer to have them mailed contact the program manager.

After your application is reviewed, you will be contacted for an interview. After your board is formed for you and before your first board meeting, you will meet with every every board member.

For one year, you and your board will work on critical issues facing your business. Meetings are held every 2-3 months with individual advisor contact between meetings.

VOLUNTEER ADVISORS ARE NEEDED!

Fill out the Volunteer Application (PDF) form, print, sign and mail or fax to the program manager. Please include a bio, resume, or CV.

FORM ACCESS AND INSTRUCTIONS

Online forms are in PDF format and can be opened and filled with Acrobat Reader or Acrobat Pro.

FOR ACROBAT READER
Open the form in Acrobat Reader and either:
1. Print form, fill out with pen OR
2. Fill in form fields on your computer, print the form, and sign by hand
3. Mail or Fax signed forms to Program Manager.
4. Unless you have Acrobat Pro you will be unable to save file to your computer.

FORM TIPS
1. Make your answers complete, but as brief as possible. If you are filling out on the computer, the form fields limit number of characters.
2. If you need more room for any form answers, write on the reverse.
3. You can use your tab key to navigate form fields.
